how to identify which monitor ac adapter should i buy for replacement?

my ac adapter is broken when it accidentally fall from stairs.
the adapter model called yuda ac adapter YD-1203A

im trying to find on online shop http://www.lazada.com.ph/shop-computer-adapters/
but i dont know which one should i look for: input,output,size, etc.

For input, all you need is something that matches your wall outlet. In the US that's 120V AC 60Hz. In Europe that's 220-240V AC 50Hz. The one you show us is 110-240V 47-63Hz so it works anywhere. That's not necessary, of course.

For output, if that's what you had then you need to match it. 12V DC at 3A is what you'll need. You need to match both the voltage and the current.

Then there's the symbol underneath that which indicates the outside of the plug is negative and the inside is positive. You need to match that.

Lastly, you need the output plug to physically match. They're somewhat standard, though, so this is typically not too hard. If it doesn't match then there are adapter kits which can help, so long as the above matches.
