Z77 Extreme 4 + Palit GTX 1060 6GB No Display / Blank Screen

Hi guys,

I am having a lot of trouble installing my new graphic card and i am running out of ideas. Need your help as this is getting quite frustrating.

My System;
ASRock Z77 Extreme 4 ( Bios 2.90M )
I7 3770K ( Default Clocks )
GSkill 16GB 1866 mhz ( 8GB x 2 )
Sandisk Extreme Pro 480GB.
CM GX 750w PSU1
EVGA 430W PSU2

Asus HD7950 GFX1 ( Works Fine )
Palit GTX 1060 6GB GFX2 ( Doesnt Work )
AMD HD4670 GFXSpare ( Works Fine )
Onboard GFX ( Works Fine )
Windows 7 64Bit Old Install.
Windows 10 64Bit New fresh Install. Same results.

Problem:
When using Palit GTX 1060 ( DVI cable connected to Palit ) i get no bios screen, motherboard shows Debug 62, i can hear windows startup sound, keyboard numlock works.

When I choose onboard gfx as primary ( cable connected to motherboard ) and leave Palit still connected i can get into windows but Palit shows a yellow exclamation mark next to it in device manager entry shows as Nvidia Geforce GTX 1060 6GB and Windows error code 43.

Whats i ve tried so far.

Tried different psus - > Same results.
Updated and used bios for motherboard 2.5, 2.7, 2.9, 2.9M -> same result.
Removed motherboard from case and running with bare minimum basic stuff - > Same result.
Tried with each memory stick in different slots with XMP Profiles selected - > Same result.
Re-Seated cpu few times -> Same result.
Tried using DDU to clean graphic drivers in safe mode few times - > Same result.
Tried changing Primary Adapter in Bios to PCI-X and manually changed to Gen3 specs for both PCI-X slots - > Same result.
CMOS reset / Battery taken out / left it out for few hrs - > Same result.
Pulled power plug out - Press on power on button to discharge -> Same result.
Tried Different DVI Cable ( Single / Dual Link ), different hdmi cable -> Same result.

Being building my own computers for years and this is the first time i am having a lot of problem installing one hardware lol.

Any inputs before i request a RMA for the card greatly appreciated.

This honestly sounds like the GPU is just dead. Could try to reinstall the drivers, but I'm %95 sure you tried that. Error code 43 is a b*tch. I had it on an old 8400M GT laptop, and after research I was sure that the GPU was just pretty much dead. So I'm going to guess that's what happened here too.

Yes i have tried reinstalling drivers many times in many different ways. I saw from another post that nvidia stopped doing analog signal on dvi output so friend is bringing a different monitor to check if thats the case as i do get sound but no signal. We ll see what happens.
