Os installing on D: and not C: and with multiple partitions

I had to reinstall windows with my usb windows 10 home. When I do this windows 10 installs on D: instead of C: like it did when i first built the pc. And before it all installed into one partition and now it does 5. Can anyone PLEASE help me

do you have 2 hard drives?

can you right click start button
choose drive management
show us a screen shot (upload to imgur and show link here)

did you do a clean install?
