Random Crashes, resolution changes, usually during games.

Hi,
so i keep getting these random game crashes whilst playing games such as WoW, fortnight and overwatch to name a few.
My game will freeze for a second or so, then the game will crash, and my monitors (using a Samsung Tv and acer monitor as secondary) will switch displays and the resolution will change on my primary monitor on which the game is being played (the samsung tv) then i will get an error message. After about 3-5 minutes the resolution and monitors will switch back. It's been happening for a while now and is becoming increasingly frustrating, anyone have any idea what this might be down to?
