LG Warranty Details Help

Hey folks;

Kinda desperate for the detailed warranty statement/PDF/pictures of the booklet for LG displays. Specifically, I'm looking to get my 38UC99-W repaired through my credit card company's extended warranty program, and they need the actual legalese (which, despite three hours of searching, is nowhere to be found online or through customer support). Does anyone happen to have this documentation for an LG monitor (I'm sure the verbiage is standard) and would be willing to throw some pictures up online, or link me to a PDF that I haven't been able to find?
