actually the problem I was thinking of was with the cable modem using the intel puma 6 chipset.
you can google your modem name + the word chipset to find out what chip it is using.s
you should try to isolate the problem down.
I would try a simple ping script to see if you can see the lag outside of your game.
if you do, then you would boot into windows safe mode with networking and run the script again to see if you still see the lag in safe mode. if you see the problem in both cases the problem is outside your machine, if only in windows full boot then something on your machine is causing the lag. some service using the network at intervals.
generally you would do a sniffer trace and look at the out put. but most people would have problems doing that.
you might run a cmd.exe or powershell as an admin then run
your ping to some server test. when you see a lag, go to another cmd.exe window and run
netstat.exe /b
it will show the name of the .exe that are currently making network connections. or ones that have just closed connections. it might help identify what is causing the lag if it is coming from your own machine.
here is microsoft's network sniffer:
https://www.microsoft.com/en-us/download/details.aspx?id=44226
(there is a large learning curve if you have never used a sniffer)
bugs in network drivers could cause errors that trigger timeouts
virus scanners can cause delays when data is downloaded.
firewalls can reject connections
you can run out of connections and a program might have to wait for a free connection.

johnbl :
what router are you using? certain router versions use a chipset that have a similar problem.
i would make sure you don't have something like onedrive backing/sycning up your files every 5 minutes.
