sorry to tell you but the gpu appears to be dead and you should thiink about a replacement as soon as possible as if you keep it in it can eventually damage other hardwares in the system and cause unwanted expensive .. i suggest you get a more modern card such as gtx 750 ti and above or a r7 265 and above for optimal performance in any games
