Here is a video of the crash.

After a few hours into a game, this crash occurs. It has done so since 2012, and all through every single driver revision and operating system since. It is definitely a hardware issue.
PCIe timings maybe? Someone said it was due to lack of voltage, tried under clocking the card with no results. I know many of you are going to say RMA it, but Galaxy lost record of my 3 year warranty extension that I registered and refuses to replace it although it was defective when purchased.
Overheating is also not an issue. 22 c idle temps, 60 - 70 load.
Anyone have a possible solution?
Edit:
PC Specs:
Galaxy GeForce GTX 680 2GB
12 GB DDR3 @ 1600 MHZ
Asus Z77 Motherboard
750w Thermaltake PSU
500 GB + 620 GB HDD
Intel i7 3770 @ Stock
